Summary:
This estate planning service allows you to designate a person you as your agent to make decisions pertaining to your healthcare treatment in the event you are incapacitated and/or not mentally competent to make your own decisions; it also allows you to limit your agent’s authority on specific matters, as well as provide some default instructions regarding how you would like medical treatment to be rendered in specific situations. Lastly, although you are designating an agent to make such decisions, the primary physician you elect will be the one to make the ultimate determination on whether or not you are incapacitated or not mentally competent.
What’s Included:
This purchase includes one Advance Healthcare Directive for an individual. Two purchases are necessary if you wish that your partner/spouse also set up an Advance Healthcare Directive.
